This scan, in numbers

Two colours, one disease.

Every one of her reports gives sizes for the red and no size at all for the blue — which is why her oncologist talks about the red, and he is right that it is the part to measure and act on. But the blue is not a separate thing sitting outside the tumour. Hers is an IDH-mutant astrocytoma, and in this type tumour cells are known to spread through tissue that never takes up dye; the international response rules written for it treat non-enhancing tissue as disease to be assessed, not as background. What these scans cannot do is say how much of her blue is tumour and how much is swelling and scarring from surgery and radiation. So: red is what gets measured and treated, blue is not proof that nothing is there, and neither outline is the true edge of the disease.

This area doesn't take up the dye, so the reports don't give it a size — but it isn't nothing, either. It's a mix the scan can't fully separate: some tumour, some swelling, and some healing from surgery and radiation. We watch it over time rather than measure it.
